At your first appointment with me, I will have you fill out a confidential health history form. This will inform me of your current and past health status in order to ensure a safe massage experience. This information helps me determine how massage might affect and influence you physically and if there are any medical reasons why massage cannot be done safely at the present time. If you’d like, you may print a copy of the Health History Form to complete in advance and bring to your appointment.
For every three referrals which result in three new clients receiving a session, I will give you a gift certificate for a complimentary one hour session.
I accept cash or personal check for payment of your session. Payment is due at time of service. A $30 fee will be charged for returned checks.
